1. INTRODUCTION

These Terms and Conditions (“Terms”) govern your access to and use of One Step Center’s comprehensive rehabilitation and recovery services, facilities, website, mobile applications, and all related platforms (collectively, the “Services”). One Step Center (“we,” “us,” “our,” or the “Company”) operates as an integrated rehabilitation facility providing medical treatment, mental health services, addiction recovery programs, vocational training, and transitional housing support.

By accessing or using our Services, you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree to these Terms, you may not access or use the Services.

One Step Center reserves the right to modify these Terms at any time in accordance with applicable laws and regulations. Any changes will be effective immediately upon posting the updated Terms on our website and providing notice through our patient portal. Your continued use of the Services after any such changes constitutes your acceptance of the new Terms.

2. DEFINITIONS

For the purposes of these Terms:

2.1 “Client” or “Patient” refers to any individual who registers for, applies to, or receives any of the Services provided by One Step Center.

2.2 “Treatment Program” refers to any structured rehabilitation protocol, therapy regimen, recovery plan, or care pathway designed and administered by One Step Center’s healthcare professionals.

2.3 “Facility” refers to any physical location operated by One Step Center, including but not limited to the main rehabilitation center, transitional housing units, vocational training centers, and affiliated medical offices.

2.4 “Healthcare Professionals” refers to licensed physicians, nurses, therapists, counselors, social workers, and other clinical staff employed by or affiliated with One Step Center.

2.5 “Personal Data” refers to any information that identifies or can be used to identify a Client, including but not limited to medical records, contact information, and payment details.

3. ELIGIBILITY AND ADMISSION

3.1 Eligibility Criteria

To be eligible for our Services, you must meet the following criteria:

a) Be at least 18 years of age, unless enrolling in our specialized adolescent programs with parental or guardian consent; b) Complete our comprehensive intake assessment; c) Meet the medical and psychological criteria for admission as determined by our healthcare team; d) Not pose a danger to yourself or others as determined during the initial assessment; e) Provide accurate and complete personal, medical, and insurance information.

3.2 Admission Process

The admission process includes:

a) Initial screening and assessment conducted by qualified healthcare professionals; b) Medical and psychological evaluation; c) Insurance verification and financial counseling; d) Development of an individualized treatment plan; e) Signing of all required consent forms and agreements; f) Completion of the admission documentation package.

3.3 Right to Refuse Service

One Step Center reserves the right to refuse admission or service to any individual if:

a) The individual does not meet our clinical criteria for admission; b) We determine that our facilities cannot safely or adequately meet the individual’s needs; c) The individual refuses to comply with essential program requirements; d) The individual provides false or misleading information during the admission process; e) The individual has previously violated these Terms in a manner that resulted in termination of services.

4. SERVICES DESCRIPTION

4.1 Integrated Rehabilitation Services

One Step Center provides a comprehensive range of rehabilitation services, including but not limited to:

a) Medical Detoxification Services: Medically supervised withdrawal management under the care of licensed physicians and nursing staff, with 24/7 monitoring and appropriate medication protocols.

b) Substance Use Disorder Treatment: Evidence-based therapies including cognitive-behavioral therapy, motivational interviewing, contingency management, and 12-step facilitation, provided in individual and group settings.

c) Mental Health Services: Psychiatric evaluation, medication management, individual therapy, group therapy, and specialized treatments for co-occurring disorders such as depression, anxiety, PTSD, and bipolar disorder.

d) Physical Rehabilitation: Physical therapy, occupational therapy, pain management, and neurological rehabilitation services provided by licensed professionals.

e) Vocational Training and Employment Services: Career counseling, skills assessment, job training, resume building, interview preparation, and job placement assistance.

f) Transitional Housing: Semi-supervised residential facilities designed to support clients transitioning from intensive treatment to independent living, with various levels of structure and support.

g) Case Management: Coordination of care, referrals to community resources, assistance with legal issues, and support navigating social services.

h) Family Support Services: Family therapy, education programs, support groups, and mediation services for families affected by a client’s condition.

i) Aftercare and Continuing Support: Ongoing outpatient services, alumni programs, relapse prevention planning, and community integration support.

j) Digital Products and Resources: Downloadable materials, templates, worksheets, assessments, and other digital tools to support recovery and rehabilitation.

k) Professional Analyses and Reports: Custom assessments, evaluations, translations, and specialized analyses conducted by qualified professionals.

4.2 Treatment Programs

Our treatment programs are structured as follows:

a) Intensive Residential Program: 30, 60, or 90-day comprehensive inpatient rehabilitation with 24/7 clinical support, structured activities, and integrated therapeutic services.

b) Partial Hospitalization Program (PHP): Structured daytime treatment (6-8 hours per day, 5-7 days per week) with patients returning to their residence or our transitional housing during non-treatment hours.

c) Intensive Outpatient Program (IOP): Focused treatment sessions (3-4 hours per day, 3-5 days per week) designed to accommodate clients who are working or attending school.

d) Standard Outpatient Services: Individual therapy, group counseling, medication management, and other services provided on a flexible schedule.

e) Specialized Treatment Tracks: Tailored programs for specific populations including veterans, healthcare professionals, first responders, adolescents, and those with co-occurring disorders.

4.3 Service Availability and Modifications

a) Service availability may vary based on facility location, staffing, and client needs. Not all services are available at all locations or to all clients.

b) We reserve the right to modify, suspend, or discontinue any part of the Services at any time, with notice provided to active clients at least 30 days in advance, except in emergency situations where immediate changes are necessary for safety or regulatory compliance.

c) Treatment plans may be modified based on clinical assessment, client progress, emerging needs, or safety concerns, with client consultation whenever clinically appropriate.

d) We will make reasonable efforts to accommodate client preferences regarding service providers and schedules but cannot guarantee specific accommodations.

4.4 Immediate Commencement of Work

a) For ALL services, products, programs, and resources offered by One Step Center, work begins IMMEDIATELY upon purchase or enrollment;

b) Preparation work, resource allocation, staff scheduling, material preparation, and other necessary processes commence instantly when an order is placed or enrollment is confirmed;

c) This immediate commencement of work applies to ALL purchases without exception, including but not limited to:

  • Treatment program enrollment
  • Individual therapy sessions
  • Group therapy registration
  • Assessment scheduling
  • Digital product purchases
  • Custom analyses requests
  • Translation services
  • Housing applications
  • Vocational training enrollment

d) The immediate commencement of work is a primary reason for our strict no-refund policy, as resources have already been allocated and work processes initiated upon purchase completion.

5. PATIENT RIGHTS AND RESPONSIBILITIES

5.1 Patient Rights

As a client of One Step Center, you have the right to:

a) Receive respectful, dignified, and non-discriminatory care regardless of age, gender, sexual orientation, race, ethnicity, religion, disability, or socioeconomic status;

b) Participate in the development and revision of your treatment plan;

c) Receive complete information about your diagnosis, treatment options, and prognosis in terms you can understand;

d) Access your medical records in accordance with applicable laws and regulations;

e) Confidentiality of your personal and health information as protected by HIPAA and other applicable laws;

f) Refuse treatment to the extent permitted by law, and to be informed of the potential consequences of such refusal;

g) Receive information about the qualifications of your healthcare providers;

h) Voice grievances and receive responses without fear of discrimination or reprisal;

i) Be free from physical restraint and seclusion except in emergency situations in accordance with applicable laws and clinical protocols;

j) Communicate with persons outside the facility and receive visitors during designated hours, subject to clinical appropriateness and safety considerations;

k) Practice your religious or spiritual beliefs that do not interfere with the well-being of others or the therapeutic environment.

5.2 Patient Responsibilities

As a client of One Step Center, you are responsible for:

a) Providing accurate and complete information about your health history, including all medications, substances used, previous treatments, and current symptoms;

b) Actively participating in your treatment program and following agreed-upon treatment plans;

c) Treating staff and other clients with respect and refraining from abusive, threatening, or disruptive behavior;

d) Respecting the confidentiality and privacy of other clients;

e) Adhering to facility rules regarding contraband, visitation, communication, and scheduled activities;

f) Meeting your financial obligations for services received;

g) Providing feedback about services to help us improve quality of care;

h) Reporting any concerns about safety, quality of care, or ethics to appropriate staff members;

i) Following discharge and aftercare recommendations.

6. CONDUCT POLICY

6.1 Prohibited Conduct

The following behaviors are strictly prohibited and may result in discharge from the program or other consequences:

a) Possession or use of alcohol, illicit drugs, or unauthorized prescription medications on any One Step Center property;

b) Bringing weapons or items that could be used as weapons onto facility premises;

c) Physical violence, threats of violence, or intimidating behavior toward staff or other clients;

d) Sexual harassment or inappropriate sexual behavior;

e) Theft of property belonging to the facility, staff, or other clients;

f) Intentional damage to facility property;

g) Leaving the facility without proper authorization if in a residential program;

h) Trading, selling, or sharing prescribed medications;

i) Recording or photographing other clients or staff without explicit permission;

j) Smoking or vaping outside of designated areas;

k) Gambling on facility premises;

l) Entering restricted areas without authorization;

m) Violating the confidentiality of other clients;

n) Engaging in romantic or sexual relationships with other clients during treatment;

o) Refusal to participate in essential program activities without clinical justification.

6.2 Drug Testing Policy

a) Random and scheduled drug testing is mandatory for all clients in substance use disorder treatment programs;

b) Refusal to submit to drug testing may result in program discharge;

c) Positive drug test results will be addressed clinically rather than punitively whenever possible, but may necessitate changes to the treatment plan or level of care.

6.3 Enforcement and Consequences

a) Rule violations will be addressed through a progressive response system, beginning with verbal warnings and potentially escalating to discharge from the program;

b) Serious violations involving safety or security may result in immediate discharge;

c) Law enforcement may be contacted in cases involving illegal activity;

d) Clients who are discharged for conduct violations may be subject to a waiting period before being eligible for readmission.

7. PRIVACY AND CONFIDENTIALITY

7.1 Health Information Privacy

a) Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws;

b) We maintain a detailed Notice of Privacy Practices that explains how your health information may be used and disclosed, which is provided to all clients upon admission;

c) Release of your health information requires your written authorization except in circumstances permitted or required by law, such as medical emergencies or mandatory reporting situations.

7.2 Confidentiality of Substance Use Disorder Treatment

a) Information related to substance use disorder diagnosis and treatment is afforded additional protections under 42 CFR Part 2;

b) We will not disclose your participation in substance use disorder treatment without your specific written consent except in strictly limited circumstances as permitted by law.

7.3 Exceptions to Confidentiality

We may disclose your protected health information without authorization in the following circumstances:

a) Medical emergencies; b) Threats of harm to self or others; c) Suspected abuse or neglect of children, elderly persons, or dependent adults; d) Court orders or valid subpoenas in accordance with legal requirements; e) Program evaluation and quality improvement activities (using de-identified information); f) Other situations as required by law.

7.4 Group Confidentiality

a) While all clients are required to maintain the confidentiality of information shared in group settings, One Step Center cannot guarantee that all clients will honor this requirement;

b) You should exercise discretion regarding the personal information you choose to share in group settings.

8. FEES, INSURANCE, PAYMENT, AND NO-REFUND POLICY

8.1 Fee Structure

a) Our fee schedule is established based on the type and intensity of services provided;

b) Fees are reviewed and may be adjusted annually;

c) A current fee schedule is available upon request and will be provided during the financial counseling portion of admission;

d) Additional charges may apply for specialized services, certain medications, or ancillary services not included in standard program fees.

8.2 Insurance Billing

a) We participate with many major insurance plans but not all plans cover all services;

b) Insurance verification will be completed prior to admission, and we will provide an estimate of covered services and potential out-of-pocket costs;

c) You authorize us to bill your insurance directly for covered services;

d) You are responsible for understanding your insurance benefits, including deductibles, copayments, coinsurance, and any pre-authorization requirements;

e) Insurance verification is not a guarantee of payment, and you remain financially responsible for all charges not covered by your insurance.

8.3 Self-Pay and Financial Assistance

a) Self-pay rates are available for clients without insurance coverage;

b) Payment plans may be arranged through our financial counseling department;

c) Financial assistance may be available based on demonstrated need and available resources;

d) Scholarship funds may be available for qualifying individuals through our foundation.

8.4 Payment Terms

a) Insurance copayments and known out-of-pocket expenses are due at the time of service;

b) Self-pay clients must maintain a payment schedule as agreed upon with our financial department;

c) Past due accounts may be referred to collections after reasonable attempts to arrange payment;

d) Failure to meet financial obligations may affect your ability to receive non-emergency services.

8.5 Strict No-Refund Policy

a) ALL SERVICES AND PRODUCTS PROVIDED BY ONE STEP CENTER ARE NON-REFUNDABLE FROM THE MOMENT OF PURCHASE. This includes, but is not limited to:

  • Assessment and intake fees
  • Treatment program fees
  • Individual, group, and family therapy sessions
  • Medical and psychiatric services
  • Laboratory testing
  • Housing fees
  • Vocational training fees
  • Digital products and downloads
  • Custom assessments and analyses
  • Translation services
  • Any and all other services or products;

b) Work begins immediately upon purchase or enrollment for ALL services and products;

c) Upon submitting payment or authorizing a charge, you explicitly acknowledge and agree that One Step Center will immediately begin allocating resources, scheduling staff, preparing materials, conducting analyses, or otherwise initiating work on your behalf;

d) The immediate allocation of resources and commencement of work renders all purchases final and non-refundable from the moment of transaction completion;

e) There is NO “cooling off” period, cancellation window, or rescission period for any purchase or enrollment;

f) Even if you have not yet attended a scheduled appointment, received a deliverable, or accessed a service, work has already commenced on your behalf behind the scenes, rendering the purchase non-refundable;

g) Any prepayment for scheduled services is non-refundable regardless of whether you attend or utilize those services;

h) In the rare case of residential treatment program early departure, a partial credit (not refund) may be issued solely at the discretion of One Step Center management, subject to a substantial administrative fee of no less than 25% of the remaining prepaid amount. Any such credit is not transferable to cash and may only be applied to future services within One Step Center;

9. MEDICATION MANAGEMENT

9.1 Medication Protocols

a) All medications brought to the facility must be declared during admission and will be verified by our medical staff;

b) Medications must be stored and administered according to facility protocols, which may include staff administration or supervised self-administration depending on the treatment program;

c) Medication changes will only be made by qualified healthcare providers after appropriate assessment;

d) Requests for specific medications will be evaluated based on clinical appropriateness, not client preference alone.

9.2 Medication-Assisted Treatment (MAT)

a) We offer FDA-approved medications for the treatment of substance use disorders, including but not limited to methadone, buprenorphine, naltrexone, and disulfiram;

b) Participation in MAT requires compliance with all program requirements, including counseling components;

c) Specific protocols govern the prescription, administration, and monitoring of MAT medications.

9.3 Psychiatric Medications

a) Psychiatric medication evaluations are conducted by qualified psychiatric providers;

b) Medication management follows evidence-based guidelines specific to each condition;

c) Clients on psychiatric medications must attend all scheduled medication management appointments.

10. TRANSITIONAL HOUSING SPECIFIC TERMS

10.1 Eligibility and Application

a) Transitional housing is available to clients who have completed or are actively participating in our treatment programs;

b) Separate application and screening processes apply for transitional housing;

c) Housing placements are made based on availability, clinical appropriateness, and individual needs.

10.2 Occupancy Terms

a) Transitional housing is provided for a limited duration, typically up to 12 months;

b) Monthly occupancy fees apply and must be paid on time;

c) All residents must sign and abide by housing-specific rules and agreements;

d) Housing units are subject to regular inspections for cleanliness and safety;

e) Overnight guests are not permitted without prior authorization.

10.3 Grounds for Termination of Housing

a) Use of alcohol or unauthorized drugs; b) Failure to actively participate in required treatment or support services; c) Violent or threatening behavior; d) Repeated violation of housing rules; e) Failure to pay occupancy fees; f) Criminal activity on or off the premises.

11. VOCATIONAL TRAINING PROGRAM SPECIFIC TERMS

11.1 Program Participation

a) Eligibility for vocational programs is determined through assessment and in consultation with the treatment team;

b) Participants must meet attendance and participation requirements;

c) Some vocational programs may have specific physical or cognitive requirements based on the nature of the training.

11.2 Internship and Employment Placement

a) Completion of vocational training does not guarantee job placement;

b) Employment opportunities are subject to availability, participant qualifications, and market conditions;

c) Participants must comply with all requirements of internship sites, including background checks or drug testing if required by placement employers.

11.3 Equipment and Materials

a) Training equipment and materials remain the property of One Step Center;

b) Participants may be held financially responsible for lost or damaged equipment;

c) Safety protocols must be followed when using vocational equipment.

12. TELEHEALTH SERVICES

12.1 Availability and Appropriateness

a) Telehealth services are available for certain types of appointments when clinically appropriate;

b) Not all services can be provided via telehealth, and clinical judgment will determine when in-person care is necessary;

c) Telehealth services may not be available across state lines due to licensing restrictions.

12.2 Technology Requirements

a) Clients using telehealth services must have access to appropriate technology, including a device with video capability and reliable internet connection;

b) Technical support is limited to basic troubleshooting;

c) Clients are responsible for ensuring the privacy of their environment during telehealth sessions.

12.3 Limitations and Risks

a) Telehealth has inherent limitations, including the inability to conduct physical examinations and the potential for technology failures;

b) Emergency situations cannot be adequately managed via telehealth and may require in-person care;

c) Despite security measures, telehealth platforms may have privacy and security risks.

13. INTELLECTUAL PROPERTY

13.1 Ownership of Materials

a) All treatment materials, curricula, workbooks, forms, and other documents provided to clients remain the intellectual property of One Step Center or their respective owners;

b) These materials may not be reproduced, distributed, or used for purposes outside of your personal treatment without written permission.

13.2 Client Creations

a) Artwork, writing, or other creative works produced by clients during therapeutic activities remain the property of the client;

b) Clients may grant permission for One Step Center to display or use such works for educational or promotional purposes through a separate authorization form.

14. GRIEVANCE AND DISPUTE RESOLUTION

14.1 Grievance Procedure

a) Clients have the right to file grievances regarding any aspect of their care without fear of retaliation;

b) Grievance forms are available at all facility locations and through our patient portal;

c) All grievances will be investigated and responded to within 14 business days;

d) Appeals of grievance resolutions may be directed to the facility director.

14.2 Mandatory Arbitration

a) By accepting these Terms, you agree that any dispute arising from your treatment at One Step Center, excluding medical malpractice claims, shall be resolved through binding arbitration administered by the American Arbitration Association in accordance with its healthcare arbitration rules;

b) The arbitration shall take place in the county where you received treatment, and the proceedings shall be conducted in English;

c) The arbitrator’s decision shall be final and binding on all parties.

14.3 Class Action Waiver

YOU AND ONE STEP CENTER AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR ITS INDIVIDUAL CAPACITY AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS OR REPRESENTATIVE PROCEEDING.

14.4 Exceptions

Nothing in this section shall prevent either party from seeking injunctive or other equitable relief from the courts for matters related to HIPAA violations, unauthorized disclosure of confidential information, or other matters for which such relief is appropriate.

15. TERMINATION OF SERVICES

15.1 Voluntary Discharge

a) Clients may leave treatment voluntarily at any time, subject to any obligations under court orders or legal mandates;

b) We strongly recommend against leaving treatment against medical advice and request that clients considering premature discharge discuss their concerns with their treatment team;

c) Clients who leave against medical advice may be subject to a waiting period before being eligible for readmission.

15.2 Involuntary Discharge

One Step Center may discharge a client involuntarily under the following circumstances:

a) The client engages in behavior that poses a risk to the safety or well-being of themselves, other clients, or staff;

b) The client repeatedly violates program rules despite intervention;

c) The client requires a level of care that the facility cannot safely provide;

d) The client has not made payment arrangements after reasonable attempts by our financial department;

e) Clinical assessment determines that the client is not benefiting from the current level of care.

15.3 Discharge Planning

a) All clients will receive discharge planning services, including referrals to appropriate follow-up care;

b) Discharge summaries and continuing care recommendations will be provided;

c) Medication prescriptions will be provided in accordance with state regulations and clinical protocols, typically covering a 30-day period or until the client can establish care with another provider.

16. LIMITATION OF LIABILITY AND INDEMNIFICATION

16.1 Disclaimer of Warranties

THE SERVICES ARE PROVIDED “AS IS” AND WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT.

16.2 Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, IN NO EVENT SHALL ONE STEP CENTER, ITS AFFILIATES, OR THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, CONTRACTORS, OR AGENTS BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, INCLUDING BUT NOT LIMITED TO LOSS OF PROFITS, DATA, USE, OR OTHER INTANGIBLE LOSSES, RESULTING FROM (i) YOUR ACCESS TO OR USE OF OR INABILITY TO ACCESS OR USE THE SERVICES; (ii) ANY CONDUCT OR CONTENT OF ANY THIRD PARTY REFERRED TO BY OR THROUGH OUR SERVICES; OR (iii) UNAUTHORIZED ACCESS, USE, OR ALTERATION OF YOUR TRANSMISSIONS OR CONTENT.

16.3 Cap on Liability

ONE STEP CENTER’S TOTAL LIABILITY TO YOU FOR ANY DAMAGES RESULTING FROM ANY CLAIM OR SERIES OF CONNECTED CLAIMS SHALL NOT EXCEED THE GREATER OF THE AMOUNT PAID BY YOU FOR SERVICES DURING THE PRIOR TWELVE MONTHS OR $1,000.

16.4 Exclusions

Some jurisdictions do not allow the exclusion of certain warranties or the limitation or exclusion of liability for certain types of damages. Accordingly, some of the above limitations may not apply to you. IN PARTICULAR, NOTHING IN THESE TERMS SHALL AFFECT THE STATUTORY RIGHTS OF ANY CONSUMER OR EXCLUDE OR RESTRICT LIABILITY FOR DEATH OR PERSONAL INJURY ARISING FROM NEGLIGENCE OR FRAUD.

16.5 Indemnification

You agree to defend, indemnify, and hold harmless One Step Center, its affiliates, and their respective officers, directors, employees, contractors, and agents from and against any claims, liabilities, damages, losses, and expenses, including, without limitation, reasonable legal and accounting fees, arising out of or in any way connected with your access to or use of the Services, your violation of these Terms, or your violation of any rights of another.

17. MISCELLANEOUS LEGAL PROVISIONS

17.1 Governing Law

These Terms shall be governed by and construed in accordance with the laws of the state where you received treatment, without regard to its conflict of law provisions.

17.2 Force Majeure

One Step Center shall not be liable for any failure to perform its obligations hereunder where such failure results from any cause beyond our reasonable control, including, without limitation, mechanical, electronic, or communications failure or degradation, pandemic or epidemic, natural disaster, act of public enemy, riot, civil disturbance, or act of any government or governmental authority.

17.3 Severability

If any provision of these Terms is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary so that these Terms shall otherwise remain in full force and effect and enforceable.

17.4 Assignment

These Terms are not assignable, transferable, or sublicensable by you except with One Step Center’s prior written consent. One Step Center may assign, transfer, or delegate any of its rights and obligations hereunder without consent.

17.5 Entire Agreement

These Terms constitute the entire agreement between you and One Step Center regarding the Services and supersede all prior agreements and understandings, whether written or oral, regarding the subject matter contained herein.

17.6 Waiver

No waiver of any term of these Terms shall be deemed a further or continuing waiver of such term or any other term, and One Step Center’s failure to assert any right or provision under these Terms shall not constitute a waiver of such right or provision.
